Topic The Missing Link… SIFs, HiPos, and Hazard Wheels in Investigations

Join us as we hear from Mike Farris on “The Missing Link… SIFs, HiPos, and Hazard Wheels in Investigations”. SIF data is difficult to trend because the data points are vastly different depending on what energy source is the focus of the reported near miss. Another issue is the magnitude of the data universe. None of us can possibly experience and trend every type of SIF or Hi-Po Near Miss. We will discuss ideas that link serious near miss data with the energy types, often displayed with a hazard wheel.  We will look at ways to isolate data points that can be trended to prevent future incidents.

Speaker Mike Farris, CSP, CHST, HSE Director for CECO Pipeline Services

Mike Farris has 30 years’ HSE experience in the oil and gas industry, having worked for Upstream and Midstream operators as well as contractors.  He has participated in several industry groups and conducted numerous studies into incident trends focused on SIF, Hi-Po, and STCKY awareness.
