Topic Validating Learning from Incidents and Normal Work

Investigations only reduce risk if they actually change how work is done in a way that is relevant to, and effective against, the risk revealed by the unplanned event that gave rise to the investigation in the first place. This presentation will discuss how to validate the output of the investigation analysis to ensure this objective is met. It will also address learning from success and everyday work.

Speaker Norman W. Ritchie (vPSI)

Norman is a Mechanical Engineer with 40 years of experience in project and risk management, largely in oil and gas. Norman co-founded vPSI Group, LLC, in 2002 to provide consulting and training in performance measurement and improvement in risk, safety, human performance, and organizational learning. He was principal developer of the 2Box and Human Analysis (HPA) techniques for the analysis of incidents involving human performance issues and holds a broad US patent for online Job Safety Analysis Systems.

Norman was a contributing author to the 2020 “One Percent Safer” book project and served as a Non-Executive Director of the One Percent Safer Foundation. He was named American Society of Safety Professionals 2020/21 Safety Professional of the Year for Texas, Oklahoma, Arkansas, and the Gulf Coast, and is a Society of Petroleum Engineers Distinguished Lecturer in Human & Organizational Performance.
