JOIN US on June 11th as we hear from Adrienne Young on “Adventures in Root Cause Analysis – Exploring Options Beyond Five Whys”. Five Whys is a methodology used across the globe to investigate incidents that occur– both big and small – to identify the causes of those incidents. Its broad-based appeal is that it is free and easy to use. It is also easy to manipulate and lead a team of investigators to arrive at a pre-conceived or desired outcome. Based on this potential for misuse, it may be time to look beyond Five Whys to other established or emerging root cause analysis (RCA) methodologies which may yield more objective outcomes and better risk management results.
ADRIENNE YOUNG, BSChE, is a Senior Consultant at BSI America Professional Services with over 25 years of practical environmental, health, and safety (EHS) experience in the specialty chemicals industry, including site-level and director-level leadership roles. She partners with clients to design, implement, and continuously improve their EHS processes to make it easier for them to comply with applicable EHS laws/regulations, conform to international/industry standards, and maintain operational resilience. Ms. Young is a certified lead auditor for ISO 9001, ISO 14001, ISO 45001, and RC14001/RCMS; is a trained RCA investigator; and has experience with OSHA VPP.
